The title is strange and almost a pleonasm : the number of crashes not caused by human errors is in fact limited.

Wait a minute… These are supposed to be "human error" accidents, but on the first one, the Captain's ADI was broken… I mean, faulty equipment CAUSED the crash, but the First Officer SHOULD'VE taken control, that's the only "human error" I see, lol…
