The harnesses and tethers were not designed for use in a helicopter, they were designed for the construction industry. Such a trivial matter when considered without the bigger picture in mind, to most people would be fairly insignificant and not likely to cause any kind of risk to life. It's not the first time though that a small seemingly insignificant detail that at the time was probably seen as purely a 'formality' has lead to the loss of life of one or more people. It's quite strange that the fuel shutoff lever in the position it was it never had any kind of detent to prevent an unintended movement to the shutoff position. So this was an accident waiting to happen, as soon as all the stars aligned. Not a matter of if it was going to happen but when it was going to happen.
I’ve flown countless work related, photo assignments like this – over water, populated areas, twilight… Never was the helicopter occupied by anyone other than myself (secured by a harness, doors off, in the rear) and the pilot. Every preflight I explained my exact photo requirements (and approximate altitude needed to get the photos) and his safety briefing – specifically focused on egress procedures when on the ground or in the water). It didn’t hurt that the pilot was a former Army helicopter pilot and I was a former Air Force aircrew member. Sad story
